a) MAIN WAIVER OF LIABILITY FORM ( 1 per team)
b) Site #2 (Boise Indoor Arena) requires also THIS WAIVER (1 per player)
(5) Medical ReleasesNote: 2 guest players allowed.

REFEREES
Referees at this event will be US Indoor Association Certified. A USindoor sanctioned referee clinic was conducted by Said Sanhoury (USIndoor Director of Officials) and offered to referees in the area on June in Boise, ID.
9 referees were certified! This is more than enough to conduct this tournament!
