    1. PLAYER APPAREL (for parents)

• (1.1) Lotto Uniform Package Order Form (send to your team manager)


• (1.2) Optional Apparel - Real Boise Store Catalog (mark items, print & give to team manager)

              2. TEAM LEAGUE ENTRY  (for team managers)

• (2.1) District League Team Entry Form & Field Inventory Form 

• District 1 Teams
• District 3 Teams
• Other Districts Teams (contact our club @ 208-713-8924)

• (2.2) INDEPENDENT TEAMS ONLY: Financial Agreement Form (contact our club)

• (3.1) Provide your team manager with a copy of player's birth certificate

• (3.2)Medical Release and Real Boise Player Registration Form (combined)

• (3.3) Financial Documents
• Financial Agreements/Commitment Forms (ask your team manager which one if in doubt):
• Level 2 Commitment Form OR
• Level 3 Commitment Form OR
• Level 3 Commitment Forms (for High School Age Brackets)


• (3.4) Real Boise Financial Aid Information and Scholarship Application

    4. COACHES/MANAGERS/VOLUNTEERS REGISTRATION and ADDING NEW PLAYERS TO EXISTING ROSTERS (for team managers)

• (4.1) Coach/Manager/Volunteer Add/Release Form (VRD)  
• (4.2) Player Add/Drop Form (to add or drop players to or from an existing roster)
